Recovery from crashed docs loop run (2026-03-12)

alph-docs-loop.ps1 -RunAll -Parallel\ run processed 29 batches. The outer process was killed while 5 batches were still running. Most Phase 1 fixes were already merged via PRs #61-#66.

New fixes in this PR (4 commits):

• playbooks-p1-chunk-1 (5 issues) — portal walkthrough path corrections
• playbooks-p2-chunk-4 (24 issues) — powershell-setup improvements
• playbooks-p3-chunk-2 (6 issues) — verification testing fixes
• reference (4 issues) — evidence standards, RACI matrix, URL corrections

Files changed: 12 files, +88/-28 lines

Still pending:

• images-expected-chunk-1 (crashed mid-fix, needs re-run)
• images-expected-chunk-3 (incomplete, needs re-run)
• maintainers-local-chunk-1 (investigating root cause of 5 consecutive failures)
