Add autocompletion and hover docs for TVM opcodes

Summary

• Add TVM opcode autocompletion in the Monaco editor with documentation, stack effects, and snippet support for operands
• Add hover documentation showing opcode details (gas, category, stack signature)
• Dynamically load Fift built-in words from the interpreter for completion suggestions, respecting the stdlib setting
