Fix signout link on error pages

The signout link used href="#signout" which appends a hash to the current URL instead of triggering Portal's signout handler. On the 404 page this navigates to /404/#signout and does nothing.

Changed to href="javascript:" which is the pattern used by Pico, Ghost Docs, and all other official themes.
