Dynamic Gait Index (DGI) - patient intake data

Here is the PDF of the public domain Dynamic Gait Index (DGI) - the Therapist scores the 4-item Likert scale with their subjective judgement of the Patient's performance of each item.

The aggregate score is a valid and reliable predictor of Falls Risk.

Brian Werner's desire is to show the correlation between FYZICAL's CTSIB and the DGI.
Note: The BodiTrak was asked to provide this functionality but they were unable to provide.

Note: this should be a toggle so each therapist is not forced to input DGI data each time

This falls outside of the CTSIB assessment, this would be its own assessment. We can calculate this trend within the Profile Inter-assessment trends, a future feature that I will be designing with VisualPT 2.0.

I can make a basic form input to start, but this would complicate our goal of capturing this data with the camera instead of the "pen and paper" solution you linked
